Einleitung
Der Begriff Aszites beschreibt die pathologische Ansammlung freier Flüssigkeit in der Peritonealhöhle. Auf der Grundlage der Zellzahl, des Gesamtproteingehalts, des spezifischen Gewichts und des Typs der nachzuweisenden Zellen wird diese Flüssigkeit als Transsudat (rein oder modifiziert) oder Exsudat klassifiziert (Tabelle 1). Diese grundlegende Klassifikation kann letztlich sowohl bei der Suche nach der Ätiologie des Aszites als auch bei der Erstellung eines Behandlungsplans hilfreich sein. Chylöse oder pseudochylöse Ergüsse, hämorrhagische, biliöse und neoplastische Ergüsse sowie ein Uroabdomen sind spezifische Exsudate, die viele Kliniker gern von echtem Aszites unterscheiden (1).
Kernaussagen
- Aszites kann nach dem Typ der abdominalen Flüssigkeit klassifiziert werden. Meist handelt es sich um ein reines oder modifiziertes Transsudat oder um ein Exsudat. Diese Klassifikation unterstützt die Einengung der sehr umfangreichen Liste von Differenzialdiagnosen der primären Ätiologie.
- Aszitesflüssigkeit sollte gewonnen und labordiagnostisch analysiert werden, um eine Klassifikation durchzuführen. Ohne umfassende weiterführende diagnostische Abklärung führt diese erste Klassifikation aber nur selten zur endgültigen Diagnose.
- Kongestive Herzinsuffizienz, Neoplasien, feline infektiöse Peritonitis (FIP) und Lebererkrankungen gehören zu den häufigsten Ursachen von Aszites bei Katzen.
- Eine therapeutische Abdominozentese kann in vielen (aber nicht allen) Fällen von Aszites zu einer wirksamen Linderung von Beschwerden führen. Empfohlen wird eine spezifische Behandlung der primären Ursache.
