Exacerbation of RAO or heaves results from the inhalation of organic dust present in hay and bedding by susceptible horses. The mechanisms by which antigen exposure causes lower airway obstruction and inflammation remain poorly defined, but alterations in both innate and acquired immune responses have been observed in affected horses. Corticosteroids are potent anti-inflammatory drugs that remain the most effective treatment for the control of RAO.
Systemically administered corticosteroids are rarely used for prolonged therapy in horses, because of the adverse effects that have been associated with their use. They include adrenal suppression, laminitis, hepatitis, altered bone metabolism, and bacterial pneumonia. Inhaled corticosteroids have revolutionized the treatment of asthma in people, and are now commonly used for the treatment of equine inflammatory lung disease including RAO. Inhalation therapy is well suited to corticosteroid administration because of the abundance of glucocorticoid receptors on both bronchial epithelial and vascular endothelial cells.
