Chiari-like malformation and syringomyelia (CMSM) are a disease complex commonly recognized in Cavalier King Charles spaniels (CKCS) and the Griffon Bruxellois, but present in many small breed dogs.1,2 Prior to imaging with MRI, this condition was difficult to diagnose and was unrecognized in veterinary medicine. Since its description in 2000, we have struggled to understand the clinical significance of the MRI findings, particularly in CKCS because of their many other MRI abnormalities including Primary Secretory Otitis media (PSOM). This is compounded by the pain syndrome associated with the disorder, which could manifest in a variety of ways that are subject to the interpretation of owner and veterinarian.
