Le tissu de granulation exubérant est une complication classique lors de cicatrisation par seconde intension. La gestion initiale de la plaie est capitale. Il faut obtenir une phase inflammatoire intense et de courte durée. Il est important d’éviter toute source d’inflammation chronique, l’exploration de la plaie peut être nécessaire. Le traitement est l’exérèse chirurgicale. Le tissu exubérant doit être excisé de sorte que le lit du tissu de granulation soit en dessous des marges cutanées. Une fois l’excision réalisée il sera important de prévenir les récidives en évitant les contaminations bactériennes et le mouvement. Au besoin une greffe cutanée peut être nécessaire.

Summary
Exuberant granulation tissue is a common complication of healing by secondary intension. The initial wound management is crucial. It is important to obtain an intense and short inflammatory phase. It is necessary to avoid any source of chronic inflammation, exploration of the wound may be necessary. The treatment is surgical excision. The exuberant tissue should be excised so that the bed of granulation tissue is below skin margins. After excision performed it is important to prevent recurrence and avoid bacterial contamination and movement. If required a skin graft may be necessary.

Lors de cicatrisation par seconde intention, la formation d’un tissu de granulation exubérant est une complication fréquente chez le cheval. Les plaies au niveau des membres forment facilement un tissu de granulation exubérant.
Le tissu de granulation exubérant dépasse des marges de la plaie, son aspect a une teinte rosée avec de nombreuses irrégularités (Figure 1). Il se caractérise par une inflammation active, une production cellulaire persistante, par un délai de contraction et une non épithélialisation.
Le tissu exubérant est fréquent au niveau des membres alors qu’il est rare au niveau de l’abdomen ou du thorax. Cela est vraisemblablement lié aux mouvements importants au niveau des membres ainsi qu’à la faible couverture vasculaire et donc par un apport en oxygène plus réduit. [...]
