Treatment of coronoid disease using various types of osteotomies has a long tradition in veterinary medicine. Indications for such techniques can be divided into (1) preemptive treatment at an early stage to slow down the progression of degenerative joint disease (DJD) in the context of fragmented medial coronoid process and nowadays medial coronoid disease, (2) treatment of medial compartment syndrome where bone on bone contact at the medial joint compartment is the most significant pathology, and (3) ancillary procedure when fixation of an ununited anconeal process is attempted. Ulnar osteotomy to improve survival of autologous osteochondral transplants at the humeral trochlea has been reported too. Osteotomies can also be divided in respect to the osteotomized bone: (1) humeral, (2) ulnar, and (3) radial osteotomies.
Proximal ulnar osteotomies and radial lengthening procedures are thought to correct radio-ulnar incongruence (RUI) within the radio-ulnar joint cup. Humeral osteotomies as well as PAUL and PURO focus on shifting load from the medial to the lateral elbow joint compartment.
