Introduzione
L’ipertensione è la malattia cardiovascolare più importante del gatto anziano ed è la malattia vascolare più importante nel gatto, rappresentando circa l’1% dei ricoveri alla NCSU dal 1990 al 1995 (Figura 1). Il suo riconoscimento e trattamento appropriato sta quindi emergendo come un componente critico nella medicina geriatrica dei piccoli animali. L’ipertensione sistemica ha tutta una serie di organi bersaglio (Figura 2) e l’esperienza dell’Autore ha mostrato che i gatti ipertesi hanno una malattia associata che interessa, in ordine approssimativo di presentazione clinica, l’occhio, i reni, il cuore e il sistema nervoso centrale (SNC) (1). Una discussione sul trattamento dell’ipertensione deve quindi essere preceduta da un dibattito sulle sue cause, sugli specifici organi bersaglio interessati e sul meccanismo mediante il quale questi vengono danneggiati. Effettuare l’esame del fundus e misurare la pressione arteriosa in tutti i gatti con oltre 10 anni d’età. In alcuni gatti non è possibile identificare una causa per l’ipertensione. Questi casi vanno considerati idiopatici e trattati in modo aggressivo per evitare danni agli organi bersaglio. È essenziale intervenire precocemente con l’amlodipina come farmaco di prima scelta, soprattutto per risparmiare l’occhio e il sistema nervoso centrale. Nella maggior parte dei felini ipertesi, il sistema reninaangiotensina-aldosterone è attivato in modo anomalo e deve essere soppresso con un ACE-inibitore o un bloccante del recettore dell’aldosterone. La tachicardia non solo contribuisce all’ipertensione, ma è anche pericolosa per il sistema cardiovascolare e la forma persistente deve essere trattata con l’atenololo.
