    Resumen

    Los antimicrobianos son una herramienta esencial dentro de la medicina porcina. Hoy en día es urgente optimizar su uso para abordar el problema de la resistencia a los antimicrobianos ya que este tema ha pasado a ser un problema de salud pública a nivel mundial. En la actualidad hay recomenda- ciones disponibles para optimizar el uso de estas moléculas que proceden de la Comisión Europea y del plan nacional de resistencias antimicrobianas (PRAN). Hasta ahora estas recomendaciones no se habían llevado a la prác- tica por los compañeros veterinarios que ejercen clínica en esta especie. En este artículo se expone un sistema de trabajo que permite compatibilizar es- tas recomendaciones con el ejercicio clínico de la profesión y que está ava- lado por el PRAN. Consiste básicamente en realizar un correcto diagnóstico microbiológico y determinar su sensibilidad antimicrobiana por técnicas cuantitativas (CMI). La información obtenida de sensibilidad antimicrobiana (MIC) de cada caso clínico se puede asociar con la pirámide de producción (granja de cerdas) de tal modo que, en un caso clínico que aparezca en el futuro, se puede utilizar el antimicrobiano más adecuado dado que existe información previa. Por último, se debe elegir el medicamento y aplicarlo con la pauta posológica adecuada bajo condiciones de campo.
     

    Abstract

    Prudent use of antimicrobials. How to approach it in practice?
    Antimicrobials are an essential tool in swine medicine. Today, it is urgent to optimize its use to address the problem of antimicrobial resistance, as this issue has become a public health problem worldwide. At present, there are recommendations available to optimize the use of these molecules that come from the European Commission and the national antimicrobial resistance plan (PRAN). Until now, the swine veterinarians have not carried out these recommendations in daily practice. This article describes a system that al- lows applying these recommendations in clinical practice. This proposal has been endorsed by the PRAN. It consists of performing a correct microbiologi- cal diagnosis, determining its antimicrobial sensitivity by quantitative tech- niques (CMI). The information obtained from antimicrobial sensitivity (MIC) of each clinical case can be associated with the production pyramid (sow farm) in such a way that, in a clinical case that appears in the future, the most appropriate antimicrobial can be used since there is prior information. Finally, the medication must be chosen and correctly applied under field conditions.
