TPLO Planning on a Plastic Bone
Radiographic Planning
- Draw the tibial long axis line (a) from a point dividing the intercondylar tubercles of the tibia to the center of the trochlear ridges of the talus. (Figure 1)
- Draw the tibial plateau axis line (b) from the cranial aspect of the tibial plateau to the caudal aspect of the tibial plateau.
- At the point of intersection of the two lines, draw a third line (c) perpendicular to the long axis of the tibia.
- Measure the tibial plateau angle (α).
- Use the chart to determine the degree of rotation required to reduce the tibial plateau slope to 5 degrees when the 24mm saw blade is used.
- Use the overlay to determine the optimal position of the osteotomy and draw a circle centered on the intersection of the tibial long axis and tibial plateau lines
- Measure D1 from insertion of the patellar tendon to the osteotomy (Figure 2).
- Measure D2 along the proximocranial aspect of the tibia to the proximal extent of the osteotomy.
- Decide whether the tibial crest will still be adequately buttressed after rotation of the segment.
- Draw the osteotomy line.
- Place two reference marks (R1 and R2) an appropriate distance apart (determined from the 24mm chart) (Figure 2).
