This presentation will define immunomodulators including immune stimulants, review those that have been and continue to be used in equine practice and present the available evidence for their efficacy.
Immunomodulators are substances or drugs that have an effect on the immune system and include immunosuppressants, immunostimulants and tolerogens. Immunosuppressants are used in the management of autoimmune and inflammatory diseases and include specific T cell (calcineurin) inhibitors, cyclosporine A, antiproliferative/cytotoxic drugs such as azathioprine, 6-mercaptopurine and cyclophosphamide and the most commonly used immunosuppressants, the glucocorticosteroids. In human medical practice bespoke specific monoclonal antibodies are also in use.
Immunostimulants have been used in the management of infections and tumours. These include levamisole, which has documented effects on B cell, T cell, monocyte and macrophage function and BCG, which continues to be used in the management of human bladder and colorectal cancer and equine sarcoid.
