Surgical candidates for the following conditions should be chosen with care. Those with chronic fibrosis and uterine damage may not be suitable candidates.
Episioplasty (Gadd’s procedure)
Used where a Caslick’s is ineffective and in mares with second- degree perineal lacerations.
Repair involves removal of 2 triangles of mucosa whose base is the cutaneous perineum and whose hypotenuse is the new dorsal vestibule. Fresh tissue exposed by removal of the triangles is apposed, thus lowering the roof of the vestibule. The muco- cutaneous junction is closed with a Caslick’s procedure.
The surgery usually takes much longer than expected, and haemorrhage may be significant.
Perineal body transection (Pouret’s procedure)
The primary indication is for mares with a severely sunken anus and most or all of the vulval lips positioned above the brim of the pelvis, where pneumovagina is not prevented by either Caslick's vulvoplasty or episioplasty. May also prevent urovagina in mild cases. [...]
