The MF is a loop of the superficial digital flexor tendon (SDFT) that wraps around the deep digital flexor tendon (DDFT) just proximal to the level of the proximal sesamoid bones within the digital sheath. With its more distal counterpart in the pastern region, it serves to maintain the two digital flexor tendons in alignment within the fetlock canal. Attached to its proximal border is a layer of synovium that divides the proximal digital sheath into two cavities that communicate only via the distal opening of the MF. The MF tapers to a very thin edge distally which is drawn into the fetlock canal with fetlock extension and is kept apposed to the DDFT even when the limb is non-weightbearing, except when torn. It is this instability that is the most useful indicator of damage on imaging as the actual site of the tear is more difficult to identify.
