    Introduction

    Le carcinome épidermoïde est la tumeur oculaire et péri oculaire la plus fréquente du cheval. Sa prise en charge peut s’avérer délicate car l’exérèse complète nécessite des marges de résection qui dans cette région ne sont pas toujours envisageables.

    Anamnèse

    Un cheval mâle castré de 12 ans nous a été référé pour la prise en charge d’un carcinome épidermoïde (diagnostiqué sur biopsie) récidivant malgré les traitements mis en place (debulking et cryothérapie).

    Examen clinique

    l’examen initial a montré une masse palpébrale inféro-temporale de 2,5 cm de long, avec atteinte du bord libre, ce dernier étant absent sur les 3⁄4 de la paupière inférieure.

    Démarche diagnostique

    Un contrôle cytologique et histologique a confirmé le carcinome épidermoïde.

    Traitement

    une exérèse de pleine épaisseur avec des marges de 5 mm a été réalisée. La reconstruction palpébrale a été basée sur une modification de la technique de Roberts-Bistner: rotation d’un lambeau cutanée dorso-latéralement au bord libre de la paupière supérieure. Le bord libre et le cul de sac conjonctival inférieur ont été recréés par une greffe de membrane amniotique congelée. La cicatrisation a été obtenue en 20 jours sans atteinte fonctionnelle significative et sans récidive sur un suivi de 3 ans.

    Discussion

    La littérature décrit majoritairement l’utilisation de traitements adjuvants variés lors de carcinome épidermoïde péri oculaire car les techniques de reconstruction palpébrale sont réputées délicates chez le cheval. Cependant des techniques de blépharoplastie sont décrites avec des résultats très intéressants.

    Conclusion

    Les techniques de blépharoplastie sont complexes chez le cheval du fait des contraintes anatomiques. Ici, la combinaison d’un lambeau cutané et de l’utilisation de la membrane amniotique a abouti à un résultat esthétique et fonctionnel satisfaisant. Le pronostic reste toutefois réservé sur ce type de tumeur.
