Make checks payable to "IVIS" and send to: IVIS, P.O.Box 4371, Ithaca NY 14852, USA. For additional instructions (dedication, anonymous donation, requests and comments), print the donation form and attach the completed form to your check (click here to print the donation form). Only USA and Canadian checks are accepted. Your cancelled check will be your receipt for tax purposes. Donations from countries other than USA and Canada should be made by credit card or wire transfer.
